The doctor opens the patient once.
CareOS composes source-linked clinical context. A microbiology result is explicitly preliminary; documented therapy remains documented therapy — not an AI recommendation.
One synthetic episode connects the product philosophy: trustworthy source context, a bounded clinician draft, a corrected result during an outage, recovery that reopens stale work, patient understanding and an acknowledged follow-up beyond the hospital.
Synthetic/pre-hospital only. This is a regression story and product specification — not clinical validation, a live integration or a measured outcome.
CareOS composes source-linked clinical context. A microbiology result is explicitly preliminary; documented therapy remains documented therapy — not an AI recommendation.
An unsigned progress-note draft references the exact source facts it used. The model has no authority to change the patient, expand tools or turn the draft into source truth.
CareOS does not pretend the missing source is negative. The legacy source remains the fallback. AI convenience is allowed to disappear before clinical truth.
The original preliminary result is no longer current. Connectivity returning is not enough to resume normal operation.
The patient-local graph detects that the draft depended on the superseded result. The unsigned AI draft becomes REVIEW REQUIRED. A signed human record would be flagged — never silently rewritten.
A different view uses the same truth philosophy: what happened, what changed, what is pending, who owns the next step, medication changes and original sources.
A minimum-purpose follow-up package is prepared. An agent may prepare it; sending requires governed human/workflow confirmation. The receiver acknowledges the process through explicit state.
This page is the human story. The repository contains the typed contracts and tests. The master plan contains the hard questions. The study page measures whether the product actually gives people time back.
